Training Agreement
High Performance Football Australia Training Program - Waiver Form
By registering and allowing my child to attend High Performance Football Australia (“HPF”) Training Sessions, I, the parent or legal guardian of the participating child (“Participant”), acknowledge and agree to the following terms and conditions:

1. Consent to Participation
I hereby give full consent and authority for my child to attend and participate in all activities organised by High Performance Football Australia in connection with these Training Sessions. I acknowledge that football is a body contact sport and that participation involves inherent risks, including but not limited to the risk of personal injury. I understand that my child and other participants are minors who may not exercise mature judgment at all times.

​2. Assumption of Risk
Participation in HPF activities is undertaken entirely at the Participant’s own risk. While due care is taken by HPF and its staff, I acknowledge the inherent risks of physical injury involved in such activities. I release and discharge HPF, its staff, contractors, volunteers, and affiliates from any liability for injury, loss, or damage sustained by the Participant in connection with any HPF activity, to the fullest extent permitted by law.

3. Medical Consent and Liability
In the event of an accident or injury, I authorise HPF staff to administer first aid and/or seek emergency medical treatment on behalf of my child. I accept full responsibility for any medical treatment or emergency service costs incurred. I warrant that my child does not have any medical conditions that would prevent safe participation in the program, except as disclosed in writing to HPF prior to participation.
I agree to notify HPF in writing of any existing or future medical conditions or medications affecting my child. I indemnify HPF and its representatives from any claim, loss, or damage arising from my failure to disclose such conditions or medications.

4. Uniform and Equipment Requirements
Participants are encouraged to wear the official HPF training gear, available for purchase through Ultra Football Clubzone: https://www.ultrafootball.com/pages/ultra-football-club-store. Alternatively, players may wear neutral training attire (e.g., club uniform or suitable athletic wear). No other academy’s uniform is permitted.
Shin pads are compulsory and must be worn during all sessions.

5. Code of Conduct
HPF maintains a strict zero-tolerance policy regarding:
Bullying (including cyberbullying)
Discriminatory or unlawful conduct
Unsportsmanlike behaviour
HPF reserves the right, at its absolute discretion, to suspend or cancel a participant’s involvement in the program due to any such conduct by the Participant, their parent/guardian, or any associated person.

6. Cancellation Policy
We kindly ask that all programs are paid in full before starting. Please note that once payment has been made, refunds aren’t available unless approved by HPF in special circumstances.

7. Photography and Promotional Use
HPF may take photographs and/or video recordings of participants during sessions for promotional purposes (e.g., social media, website, flyers). By agreeing to this waiver, I consent to the use of my child’s image for such purposes. If I do not wish for my child’s image to be used, I will notify HPF in writing prior to the start of the program.

​8. Governing Law
This Waiver and Release of Liability Agreement shall be governed by and construed in accordance with the laws of the State of New South Wales, Australia.

9. Acknowledgment and Agreement
By ticking the box or otherwise indicating agreement to this Waiver and Release of Liability Agreement, I confirm that:
I am the parent or legal guardian of the Participant;
I have read and understood all of the above terms;
I voluntarily agree to be bound by them.
